Does Insulin Resistance Stop Weight Loss?

A viral claim says insulin resistance is the real reason you can't lose weight. Dr. Joey Munoz breaks down the mechanism and the research, and neither backs up the claim.

You saw the clip. Someone with a big following says the primary reason most people can't lose weight is insulin resistance, and now you're wondering if you need to fix your insulin before your diet can actually work. Maybe you've already tried apple cider vinegar before meals, or gone low-carb specifically to keep insulin down. Here's the problem: the claim gets the mechanism backwards, and the research on real people directly contradicts it.

The Claim: Insulin Resistance Is Why You Can't Lose Weight

This claim has a specific logic behind it, and it's worth taking seriously enough to test properly rather than dismiss outright. Insulin does slow down fat breakdown. Insulin resistant people do have higher circulating insulin. So the claim assumes: more insulin means more suppressed fat breakdown, means harder weight loss. It sounds mechanistically plausible. It's also not what happens when you actually study it in humans.

What Insulin Actually Does, in Plain Terms

Insulin is released by your pancreas when you eat carbohydrates, and its main job is shuttling glucose out of your blood and into your muscle, liver, and fat cells. It does this by signaling a transporter protein called GLUT4 to move to the cell surface and let glucose in. At the same time, insulin slows a process called beta oxidation, which is how your body burns stored fat for energy. That part of the claim is accurate: insulin does suppress fat burning while it's elevated.

Here's the piece the claim skips: insulin resistance doesn't mean insulin's fat-suppressing signal keeps working while everything else about it fails. Insulin resistance is a global reduction in how well your cells respond to insulin. That reduced sensitivity applies to the fat-breakdown suppression too. Your cells become less responsive to insulin telling them to slow fat burning, in the exact same way they become less responsive to insulin telling them to absorb glucose. The claim only accounts for half of what insulin resistance actually changes.

What Happens When You Actually Test This in People

If insulin resistance genuinely blocked weight loss, diabetics and insulin-resistant people on a calorie deficit should lose less weight than insulin-sensitive people on the same deficit. Two studies looked at exactly that.

A second, older study from 1999 approached it differently: same calorie-restricted diet for everyone, then checked whether people's baseline insulin resistance predicted how much weight they'd lose. Thirty-one obese adults followed the same deficit for 60 days, losing about 18 lbs on average. There was no correlation between starting insulin resistance and how much weight someone lost. Of the participants who failed to lose weight, roughly half were insulin resistant and half were insulin sensitive, an even split that tracks with adherence, not insulin status.

Weight loss improves insulin sensitivity. The research doesn't support it working the other way around.

What Actually Causes Insulin Resistance

If it's not carbs or sugar directly, what is it? Primarily chronic inflammation, and one of its biggest drivers is excess body fat itself. Fat tissue functions as an endocrine organ, releasing hormone-like molecules that affect the rest of your body. As fat cells grow beyond a certain point, blood supply struggles to keep pace, creating a low-oxygen environment that triggers fat cells to release more inflammatory molecules like TNF-alpha and interleukin-6.

Those inflammatory molecules directly interfere with insulin's ability to bind its receptor and trigger the GLUT4 response, which is the actual mechanism behind insulin resistance. This is why weight gain, not sugar intake or carb timing, is the primary driver: it's the excess fat tissue causing the inflammation causing the resistance. Reverse the weight gain, and insulin sensitivity typically improves as a downstream result.

It's the same foundation that works for anyone: a sustainable calorie deficit, enough protein and fiber, and resistance training to build muscle. Improve your body composition, and your insulin sensitivity improves alongside it, not the other way around.
